How a well woman Health MOT differs from a smear appointment

A well woman Health MOT is a doctor-led assessment of metabolic health, heart, hormones as context, and, on Elite, consultant gynaecology with ultrasound and a smear. A smear appointment is one test of the cervix. They are different visits. Booking a smear and calling it a well woman check leaves the rest of you unexamined. Booking Elite because you only wanted a smear buys a three-hour MOT you did not ask for.

People usually mix the two because both can happen on Harley Street, both can include a speculum, and both can feel like women's health. NHS cervical screening is already a national programme: women and people with a cervix aged 25 to 64 are invited every five years if HPV-negative (England); sooner if HPV is found. A private smear inside a Health MOT does not replace that invitation. It is one Elite component, read with a consultant examination and pelvic imaging, not a stand-alone all-clear. The well woman MOT also includes the metabolic and cardiac work that a smear appointment never did: blood pressure, ECG, lipids, thyroid, iron, glucose and a doctor who will join those results to the pelvic findings.

What a smear appointment is

Cervical screening checks the health of the cervix and helps prevent cervical cancer. On the NHS it is an HPV-first test offered to a defined age group on a defined interval. A private smear is the same kind of sample taken in a clinic that is not the national programme. It can be useful if you have missed a screen, or want the sample taken as part of a wider assessment. It still only answers a cervical question.

It does not measure blood pressure, lipids, thyroid, iron or diabetes risk. It does not include an ECG. It does not look at the ovaries or the uterus on ultrasound. It does not review a hormone profile. Calling it a well woman check is how people leave with a cytology or HPV result and a false sense that the rest of their health has been seen.

If you already know that a smear, or a coil, is the only appointment you wanted, that work sits with the sister Gynae Clinic, which is a different diary from a Health MOT. It is not the booking this article is trying to sell.

What the female Elite MOT is

The female Elite MOT is the well woman Health MOT with consultant gynaecology. It currently costs £2,095. Allow two and a half to three hours on a Thursday or Friday specialist day, when women's Elite runs, with a four-hour fast. It includes the doctor-led baseline, Advanced imaging and hormones, then a gynaecologist consultation and examination, trans-vaginal and pelvic ultrasound, a cervical smear, a mammogram from 40, CA-125, CEA, and four GP consultations over a calendar year.

Consultant gynaecology on Elite is led by Dr Hikmat Naoum, GMC 3047637. That is a clinician fact about who holds the gynae lead, not a promise that every visit is with one named surgeon. Ultrasound looks at the uterus, ovaries and pelvic organs. A smear sits beside that imaging rather than standing in for it. A mammogram from 40 looks at breast tissue, which neither a smear nor a pelvic scan can see.

The consultants page lists the senior team. You can book Elite as a well woman MOT, or read a narrower gynae screen if the question is only pelvis and smear. The Essential gynae screen is that narrower diary. Breast-only worry belongs on breast screening, where a mammogram from 40, ultrasound or MRI is chosen by age and density rather than bundled into a smear slot.

Hormones are context, not a menopause clinic

Elite includes a female hormone profile: LH, FSH, prolactin, testosterone, oestradiol, SHBG and a free androgen index. Those numbers are context for the GP and the gynaecologist. They are not a menopause diagnosis and they are not an HRT prescription.

NICE menopause guidance (NG23) is plain that, in otherwise healthy people aged 45 or over with menopause-associated symptoms, perimenopause and menopause should be identified from symptoms, without laboratory tests. Do not use FSH, oestradiol or imaging to label menopause in that group. FSH may be considered earlier, including under 40 when premature ovarian insufficiency is the question, which is a different clinical problem from a well woman screen.

This clinic is not using Elite to sell HRT. If menopausal symptoms are the reason you came, say so, because the useful conversation is then symptoms, examination and whether screening still belongs in the same visit. They do not replace NG23.

A smear does not replace NHS cervical or breast programmes

A private smear inside Elite is not the NHS Cervical Screening Programme. Keep the invitations you already receive. NHS.uk currently invites women aged 25 to 64 every five years if HPV-negative (England); sooner if HPV is found. A private sample does not mean you should opt out of the national list.

A mammogram on Elite from 40 is not the NHS Breast Screening Programme, which invites women aged 50 to 70 every three years in England, with older women able to request screening. GOV.UK is honest that screening can find something that needs more tests, and that most people called back do not have cancer. A private well woman MOT inherits that trade-off. More pictures mean more findings to explain, and some of those explanations will be that the finding is not important.

CA-125 on Elite is extra information, not an ovarian screen. NICE NG12 uses CA125 inside defined symptom-led pathways. The UK National Screening Committee does not recommend population ovarian screening. A raised marker or a cyst on ultrasound is a reason for the gynaecologist and GP to decide what, if anything, needs to happen. It is not a diagnosis.

When a smear-only visit is the honest booking

Choose a smear-only visit when the cervix is the only question, you are up to date with everything else you wanted examined, and you do not want a Health MOT.

Choose Elite when you want a well woman assessment that includes that smear as one component, alongside consultant ultrasound, metabolic bloods, an ECG and a doctor who will review the whole report.

Essential, at £495 and 45 minutes, is still a well woman baseline if you want examination, ECG and metabolic bloods without gynae imaging. Advanced, at £999, adds a chest X-ray, DEXA when indicated, lung function, fitness work, hormones and four GP visits, still without consultant ultrasound or a smear. Elite is the smear-plus-ultrasound well woman MOT. Ultimate adds MRI of the brain, MRA, whole spine and torso, currently £3,350 with gynaecology. That is a different job again.

New pelvic pain, heavy bleeding, a breast lump, or any new or worsening symptom still belongs with a GP or urgent assessment, not with a screening diary. Use 999 or A&E for emergencies.

Tips and tricks

• Decide whether the question is the cervix or the whole assessment before you book.
• A smear-only visit and the female Elite MOT are different lengths, different fasts and different reports.
• Bring previous smear results, HPV reports, mammograms, hormone tests and a medicines list.
• Fast for four hours if you have booked Elite.
• Continue regular medicines unless you were told otherwise.
• Keep NHS cervical and breast invitations even if Elite includes a smear and, from 40, a mammogram.
• Say if menopause symptoms are part of the picture, although that does not turn the MOT into an HRT clinic. The review is when a flag, a cyst or a hormone number becomes a plan.
• If you only wanted a smear after reading this, stop at that narrower diary rather than sitting through Elite.

Things to avoid

• Do not call a smear a well woman check.
• Do not skip NHS cervical or breast screening because a private MOT included one of those tests.
• Do not treat CA-125, a pelvic scan or a hormone profile as a menopause diagnosis or a cancer all-clear.
• Do not book Elite as a way of buying HRT, and do not use FSH after 45 to "prove" menopause against NICE NG23.
• Do not wait on a screening slot if you have a new breast lump, heavy bleeding or pelvic pain that is getting worse.
• If you want the well woman Health MOT that includes consultant ultrasound and a smear, review the female Elite MOT.
