MRI-only versus a doctor-led MOT

London sells two different appointments under similar words. One is MRI-only: you attend a scanner, a radiologist reports the pictures, and you leave with a PDF. The other is a doctor-led MOT: a GP examines you, bloods are taken, imaging sits inside that same clinical conversation, and someone who has seen you explains the lot. They are not the same product at two prices, and mixing them is how people pay for a torso series and then wonder why nobody listened to their chest or measured their HbA1c.

This is not a booking page titled around a city-wide scan search. It is a comparison of models. Individual providers are not named, since the models stay the same when brands change. The clinic's buying guide already maps pop-ups, hospital wellness menus, imaging-only MRI and ultra-premium executive days onto Health MOT tiers. The Health Screening Clinic has been at 117a Harley Street since 1984 and is CQC registered. Ultimate from £2,950 is the combined offer: examination, over 70 blood markers, and a protocol of brain MRI, intracranial and carotid MRA, whole spine and torso. MRI is still not a cancer all-clear, and a laboratory flag is still not a diagnosis. If you have a new, severe or worsening symptom, see a GP or use urgent care, and use 999 or A&E for emergencies, rather than waiting for a scanner.

What MRI-only usually includes, and what it quietly omits

Imaging-only full-body MRI in London is typically a scan and a radiologist's report for the pictures alone. Blood tests are usually absent, so is a hands-on doctor examination, and a dedicated results appointment with the person who examined you is absent because nobody examined you. Brain sequences, intracranial MRA, carotid MRA and whole spine are often sold as upgrades rather than as the core protocol.

That model can be the right booking if you already have recent bloods, a GP who will own the report, and a defined structural question. It is a weak booking if you wanted cholesterol, thyroid, glucose, iron, a heart tracing, lung function, or someone to join a cyst on the liver to the liver enzymes in the same conversation. MRI shows structure. It does not measure chemistry, hear a murmur, or replace a smear, a mammogram or a stool FIT kit.

Coverage varies by operator. Torso-only series miss the brain and the neck vessels. A brain without MRA misses a look at intracranial and carotid arteries. A lumbar sequence is not a whole spine. Ask what is in the slot rather than what the phrase full-body implies. This piece will not rewrite that essay.

The Royal College of Radiologists has said it recommends body scanning to investigate symptoms under a doctor, and does not recommend trying to obtain an MRI without that direction if you have no symptoms, because of overdiagnosis. The UK NSC recommends screening programmes only when the pathway does more good than harm. Unselected whole-body MRI is not one of those programmes, and paying for pictures does not move the evidence bar.

What a doctor-led MOT adds around the scan

Every Health MOT at this clinic includes a GP consultation, a physical examination, blood tests, a report and a review.

Essential at £495 is that core in 45 minutes. Advanced at £999 adds chest X-ray, DEXA when indicated, spirometry, VO2 max, hormones and four GP appointments over a year. The Elite Health MOT at £1,850 for men and £2,095 for women adds sex-specific ultrasound and tumour markers. None of those three is an MRI package, and they are the reason a scan-only day is not "the same MOT without the magnet".

Ultimate is the tier that puts MRI inside the MOT rather than selling the magnet as the MOT. From £2,950 without gynaecology, or £3,350 with it, you allow 3 to 4 hours including 60 minutes of scanning. The protocol is brain, intracranial and carotid MRA, whole spine, and torso covering chest, abdomen and pelvis. DEXA and chest X-ray are replaced by that MRI, not duplicated. Fast for a minimum of four hours because of the bloods. Four GP appointments across the year sit on this tier as they do from Advanced up.

The over 70 blood markers include the metabolic panel, iron studies, gluten-sensitivity testing on this level, CEA, and, for women, HE4, CA-125 and ROMA. Markers remain clues, which neither diagnose cancer nor exclude it. The point of combining them with MRI and an examination is that a raised protein and a quiet scan, or a cyst and ordinary chemistry, can be read together. A disc in an envelope cannot do that work.

Book the combined visit if that joining-up is the job. Book MRI-only if you already have the rest, and you accept that the report will land without a Harley Street examination behind it.

MRI is not a cancer all-clear in either model

A clear radiologist report means the pictures, that day, in those regions, did not show a structural change the reader flagged. It does not mean every tissue is free of disease, or that a mucosal tumour, a blood cancer, or a lesion below the resolution of a screening-style series is absent, and it does not replace NHS breast, cervical and bowel programmes. NICE NG12 remains a symptom pathway. New haemoptysis, a breast lump, rectal bleeding or unexplained weight loss still belong with a GP, not with a scanner diary.

Incidental findings are the other half of the same limit. Cysts, nodules and degenerative discs appear because the volume scanned is large. Some need a further investigation, and most do not change your health. MRI-only reports still contain those sentences. The difference is whether a doctor who has examined you is employed to grade them with the bloods, or whether you are the person who forwards a PDF.

Ultra-premium executive models that add PET-CT or extra CT on top of MRI sit in a higher advertised band, often several times Ultimate's combined price. More modalities are not automatically wiser screening. PET-CT is a different instrument with radiation and a different false-positive profile. This clinic does not use those extras to justify a theatre-style fee. That is a model choice rather than a moral one. Ask whether each extra picture would change a decision you are prepared to act on.

Tips and tricks

• Write down the job before you write down the magnet.
• Structure only, with a GP at home who will read the report, can match MRI-only.
• Structure plus chemistry plus an examination matches Ultimate.
• Specialist ultrasound without MRI matches Elite.
• Ask any imaging-only operator what regions are included, whether brain, intracranial MRA, carotid MRA and whole spine are core or upgrades, who reports the scan, and who you speak to if a nodule is named.
• Ask any MOT clinic the same questions, plus whether a GP examines you on the day and reviews every result in person.
• Tell the team about implants, claustrophobia and metal in the eyes before an MRI is confirmed.
• Fast for Ultimate because of the bloods.
• MRI itself does not usually need a fast unless you are given a separate instruction.
• Keep NHS screening invitations either way.
• If you have already bought a scan-only slot, you can still add a doctor-led baseline later.
• Do not assume the PDF has done the bloods.

Things to avoid

• Do not treat an MRI-only day as a doctor-led MOT. It omits the examination and the over 70 blood markers.
• Do not treat Ultimate as a cancer all-clear because the protocol is wide.
• Do not assume brain, MRA and whole spine are included in every London scan price you have seen. Those sequences are often upgrades.
• Do not book a scanner because you have a new symptom.
• Do not skip bloods because the pictures look complete.
• Do not skip a smear, a mammogram or a FIT kit because an MRI was reassuring.
• Do not choose by brand name.
• Choose by model: pictures alone, or pictures inside a MOT.
